NDIS (National Disability Insurance Scheme)

Who it's for:

  • People with a permanent and significant disability.

Requirements:

  • You need to be an NDIS participant. Make sure you have an approved plan that includes physiotherapy.

How to proceed:

  • Once you confirm your NDIS status and plan details, contact us to book an appointment. Please bring a copy of your plan to your first session.

Relevant Forms:

  • NDIS Service Agreement Form, available on the NDIS website or at our clinic.

WorkCover Claims

Who it's for:

  • Individuals who've suffered injuries or illnesses related to their workplace.

Requirements:

  • You must have reported the injury to your employer and have a WorkCover claim number.

How to proceed:

  • Prior to treatment, ensure you have a referral from your doctor for physiotherapy. Present your referral and claim details during your initial appointment.

Relevant Forms:

  • WorkCover Medical Certificate and any claim-related paperwork.

DVA (Department of Veterans' Affairs)

Who it's for:

  • Veterans and members of the Australian Defence Force.

Requirements:

  • You need a valid DVA card (Gold or White) and a referral from a General Practitioner.

How to proceed:

  • With your referral and DVA card, contact our clinic to book an appointment. Bring both items to your session.

Relevant Forms:

  • D904 Referral Form, which can be provided from your GP.

CTP (Compulsory Third Party) Insurance Claims

Who it's for:

  • Individuals injured in a motor vehicle accident.

Requirements:

  • You should have reported the accident to the relevant insurance company and have a claim number.

How to proceed:

  • Before commencing treatment, you'll need a doctor's referral. On your first visit, provide us with the referral and your claim details.

Relevant Forms:

  • CTP Insurance Medical Certificate and any additional claim-related documents.

Chronic Disease Management Plan Formerly EPC

Who it's for:

  • Individuals who suffer from a chronic medical conditions.

Requirements:

  • You need a valid care plan provided to you from you GP.

How to proceed:

  • Before commencing treatment, you'll need a doctor's referral. On your first visit, provide us with the referral and bring your Medicare card.
